Credible Witness Affidavit
The credible witness affidavit certificate may be used when the signer is present before the Notary, but without adequate ID documents. Notaries may identify the signer through the “sworn written statement” of one personally known credible witness who is personally known to the Notary. Notaries may also identify the
signer through two credible witnesses, who know the signer, and are identified through ID cards.
